- Proof of Address (Recent utility Bill)

Monthly Premium Payment Plan Clause
It warranted that the premium payment plan under this policy is on monthly basis. Any default or gap in premium payment of any kind shall render the arrangement void and subsequent payment will be deemed to be a fresh contract which shall commence immediately after the date of the new payment.
In the event of any loss within this period of monthly premium payment arrangement, the insured shall be required to pay up the balance outstanding annual premium before such claim shall be entertained or treated.
In event of cancellation of policy within this, the insured shall not be entitled to any form of refund of premium as the policy is on monthly premium payment plan.
